I'm a nurse that floats twice a week to an Ortho/Neuro floor. Chances of recovery are pretty grim when a patient is in a comatose state as a result of a TBI. None of the patients that I've been assigned to with TBIs have ever made a full recovery (So far). They either remain in a comatose state or awaken with all sorts of debilitating neurological deficits/paralysis. I'm very worried for Juan and his family

Ya, this is terrible. I'm in medicine as well and this injury is terrible and the prognosis is pretty grim. They quote 10% but that's survival. No one ever really makes any sort of meaningful recovery. This is tragic.

Wow, this is awful news. And also perplexing – how does Juan, a highly experienced cyclist, wipe out hard enough to put himself in a coma? And then who dialed emergency services if he was knocked out? I guess a passerby eventually found him? Whatever the case, quite tragic news.

Healthcare people that work in the ER know that you should never ride a motorcycle (with or without a helmet) ride a bicycle or one of those scooters that is so popular with millennials without a helmet) or fly in a helicopter.  Eventually your luck will run out and you will wipe out.  Most of the time, it's not the driver's fault but the person the clipped the motorcycle, ran the light, etc.  No room for error.  You really don't have to wipe out "hard."  Even a fall from standing can take your brain out.

But it was a solo accident and he had a helmet + protective gear.

Hope be pulls though, I cycle a decent amount and have had several friends hit, ton a bad drivers out, be safe. Still crazy he had that kind of head injury while wearing a helmet the driver must have been going preety fast.
